WASHINGTON (AP) ? The survival of President Barack Obama's health care overhaul rests with a Supreme Court seemingly split over ideology and, more particularly, in the hands of two Republican-appointed justices.
Chief Justice John Roberts and Justice Anthony Kennedy put tough questions to administration lawyers defending the health care law during three days of arguments that suggested they have strong reservations about the individual insurance requirement at the heart of the overhaul and, indeed, whether the rest of the massive law can survive if that linchpin fails.
But Roberts and Kennedy also asked enough pointed questions of the law's challengers to give the overhaul's supporters some hope. In any event, justices' questions at arguments do not always foretell their positions.
The court's decision, due in June, will affect the way virtually every American receives and pays for health care and surely will reverberate in this year's campaigns for president and Congress. The political effects could be even larger if the court votes 5-4 with all its Republican-appointed justices prevailing over all the Democratic appointees to strike down the entire law, or several important parts of it.
Not since 2000, when the court resolved the Bush v. Gore dispute over Florida election returns that sealed George W. Bush's election as president, has a Supreme Court case drawn so much attention.
The court wrapped up public arguments Wednesday on the overhaul, which aims to extend health insurance to most of the 50 million Americans now without it. The first and biggest issue the justices must decide is whether the centerpiece of the law, the requirement that nearly all Americans carry insurance or pay a penalty, is constitutional.
Wednesday's morning session was unusual in that it assumed, for purposes of argument, a negative answer to that central question. What should happen to other provisions, the justices and lawyers debated, if the court strikes down the requirement? If the justices are following their normal practice, they had not even met to take a preliminary vote in the case before all argument concluded.
Questions at the court this week showed a strong ideological division between the liberal justices who seem inclined to uphold the law in its entirety and the conservative justices whose skepticism about Congress' power to force people to buy insurance suggests deep trouble for the insurance requirement, and possibly the entire law.
The divide on the court reflects a similar split in public opinion about the law, which Congress approved two years ago when Democrats controlled both the House and Senate.
Liberal and conservative justices alike appeared to accept the administration's argument that at least two important insurance changes are so closely tied to the must-have-coverage requirement that they could not survive without it: provisions requiring insurers to extend coverage to people with existing medical problems and limiting how much those companies can charge in premiums based on a person's age or health.
Less clear was whether the court would conclude that the entire law, with its hundreds of unrelated provisions, would have to be cast aside.
The justices also spent part of the day considering a challenge by 26 states to expansion of the federal-state Medicaid program for low-income Americans ? an important feature which alone was expected to extend coverage to 15 million people and which no lower court has rejected. The conservative justices appeared open to the states' argument that the expansion is unconstitutionally coercive.

Solicitor General Donald Verrilli Jr. took a few seconds at the end of the Medicaid argument to make a final plea for the court to uphold the entire law, which he said would "secure the blessings of liberty" for millions of Americans by providing them with affordable health care.
Verrilli told the court that Congress had made a policy decision to fight the high cost of medical care through the new law. "I would urge the court to respect that judgment," he said.
Paul Clement, the lawyer for the states challenging the law, answered that it would be a strange definition of liberty to make people who may not want it buy health care insurance. And he called Congress' threat to cut all Medicaid funding from states that refuse to expand the program "a direct threat to our federalism."

This trial is registered with ClinicalTrials.gov, number NCT00363311.
FINDINGS: Between Aug 10, 2006, and March 26, 2007, we randomly allocated 302 participants, of whom 289 (96%) had at least one biopsy procedure after baseline and were included in the primary analysis. By 3 years, 54 (38%) of 144 men in the dutasteride group and 70 (48%) of 145 controls had prostate cancer progression (pathological or therapeutic; hazard ratio 0.62, 95% CI 0.43-0.89; p=0.009). Incidence of adverse events was much the same between treatment groups. 35 (24%) men in the dutasteride group and 23 (15%) controls had sexual adverse events or breast enlargement or tenderness. The tournament did indeed put Serena on the stadium for the night session, and she found herself in a struggle from the start.
The points were long, with Williams unable to overpower her opponent for a change. She matched a career high with 20 aces in the fourth round but had only four against Wozniacki and lost serve three times.
It was a momentous victory for the No. 4-seeded Wozniacki, whose top ranking last year drew criticism because she hasn't won a Grand Slam title.
"If people talk about me or not, I mean, to be honest, as long as I win, I'm happy," she said.
She had lost her previous three meetings against Williams.
"Serena has won so many Grand Slams and so many tournaments, something that I would love to achieve one day," Wozniacki said. "So to beat someone like her that I know that never gives up is definitely something that means a lot to me."
Williams, seeded 10th, was playing in her first tournament since January after being sidelined by a left ankle injury. She hasn't won a title since August.
There's still one Williams in the tournament: Venus plays No. 5 Agnieszka Radwanska in the quarterfinals Wednesday.

Spoiler Alert: Don't read this season premiere recap if you haven't yet watched the episode as there will be spoilers. Wait, you're still reading? OK, but don't get mad if we talk plot points.
Now where were we?
Last we saw him, manly enigma Don Draper was dropping loving, intelligent Faye for an impulsive proposal to his hot, young, good-with-kids secretary Megan; beleaguered bombshell Joan was attempting to deal with a surprise pregnancy; and the rest of Sterling Cooper Draper Pryce was trying to stay in business while having a few drinks.
And now? Well, the season 5 premiere began with an expected splash...

...as members of a rival advertising firm drop water-filled sacks on the heads of a civil rights-minded picket line -- setting up a neat distinction between the antics of the all-white boys' club boardrooms of Madison Ave. and the sweeping cultural changes taking place out in the streets. And completely missing the point, Sterling Cooper Draper Pryce responds to this racist pranksterism with a joke of its own: an ad announcing that it is an equal opportunity employer. Except, as far as they know, they ain't making any changes. Yet.
But this two-hour premiere written by Matthew Weiner offers plenty of conflicts to dig into: Megan not only married the boss but she's moved from secretary into creative work; the continuing backward behavior of the Mad men toward women, African-Americans and homosexuals is putting them more and more out of step; and, well, a whole host of various marital and family issues. All that, plus we first got the indelible images of Megan singing goofy French song "Zou Bisou Bisou," and then later cleaning the house in her skimpy black underwear ... why exactly? Because she's just so angry at Don, that's why!
Still, it's nothing a shag on the carpet can't fix -- for the moment anyway.

So that's the broad scope: The times are a-changing and the skirts are a-rising, but what's going on with Don (Jon Hamm), Peggy (Elisabeth Moss), Pete (Vincent Kartheiser), Roger (John Slattery), Megan (Jessica Par) and the rest?
Don Digs Domesticity: Newly married and about to turn 40, weekend dad Don dotes on his visiting kids by making breakfast in his huge family-style apartment and using his chiseled straight-arm in place of a seatbelt. Gone is the gloomy pad of last season, Don and Megan live in a colorful high-rise apartment that's just perfect for a groovy surprise 40th birthday bash -- which Don endures even as Megan dances and purrs through a French song that both embarrasses him and titillates all the assembled men. But Don doesn't like surprises, so Don goes to bed drunk and angry. You're welcome.
Joan Alone: Home with her baby and her visiting mother, Joan needs to get back to work. She snaps at her mom and seems miserable being away from the office -- though thankfully she doesn't seem to dislike her baby. So there's that, but it's only when she comes to visit Lane that she really starts to feel needed, important and where she belongs.

Pete Is Growing Up: Sure, he'll always be a bit of the grasping creep who got Peggy pregnant and feels slighted by everything, but Pete is shaping up to be the Man in Charge: He's working hard to bring in clients, avoiding excessive drinking and smoking and developing into a thoughtful husband and father. And if that means butting heads with Roger to get a better office, then so be it. Pete will not be denied.
Roger, Over and Out? Roger is definitely struggling. But he's at his funniest when times are tough, and the wounded lion spends the episode coaxing the secretaries to sit in front of his office, fending off Pete's advancements and insulting his own trophy wife as he heads off on an early-morning wild goose chase to steal a client.
And Betty -- Wait, Where's Betty? We don't see January Jones' Betty this episode, but do get a look at the creepy Gothic gloom palace she lives in with her new husband, Henry. "Give my love to Morticia and Lurch," quips Don as he drops the kids off, and we kind of love him for it.

There's more potential trouble bubbling under the surface as Lane struggles with money problems and a strange flirtation with a woman over the phone, Peggy works too hard on the Heinz campaign and then shoots her mouth off to Don at the party and Ken (Aaron Stanton) ... well, he always seems perfectly happy and unchanged by the world. Good for you, Ken.

BANGKOK (AP) ? World stock markets rose Tuesday after comments by Federal Reserve chief Ben Bernanke suggested he thinks more needs to be done to help spur the U.S. economy.
Benchmark oil hung above $107 per barrel. The dollar dropped against the euro and the yen.
Stocks were higher in early European trade. Britain's FTSE 100 rose 0.5 percent to 5,932.07. Germany's DAX gained 0.9 percent to 7,141.12 and France's CAC-40 rose 0.8 percent to 3,529.64.
Asian stock markets posted solid gains throughout the day, led by Japan's Nikkei 225 index. The benchmark jumped 2.4 percent to close at 10,255.15, its highest finish since a devastating earthquake and tsunami on March 11, 2011.
South Korea's Kospi rose 1 percent to 2,039.76 with heavy industrial shares helping to push the benchmark higher. Hong Kong's Hang Seng climbed 1.8 percent to 21,046.91 and Australia's S&P/ASX 200 added 0.9 percent to 4,301.30.
Bernanke told an audience at the National Association for Business Economics that the U.S. job market was still weak despite recent signs of improvement. That could mean he believes the Fed needs to continue to prop up the economy by keeping short-term interest rates near zero and perhaps by buying more bonds later.
The Fed has embarked on two previous rounds of bond-buying, most recently in late 2010, known as quantitative easing. The idea is to drive down long-term interest rates and encourage investors to buy stocks. The second round ignited a 28 percent Wall Street rally in eight months.
The mere thought that a third round of bond-buying, dubbed QE3 by industry insiders, might be possible was all it took for markets to respond.
"Bernanke said that the Fed has to maintain the regime of loose monetary policy, and the market interpreted this as a chance for QE3," said Francis Lun, managing director of Lyncean Holdings in Hong Kong. "That's why the stock markets rallied," he said, referring to Wall Street. "And that carried over to Asia."
The dollar weakened against most major currencies since traders interpreted Bernanke's comments to mean that interest rates will remain near zero. Lower rates tend to weigh on a currency by reducing the returns investors get from holding it.
That helped support prices for commodities since they are traded in dollars and considered more of a bargain for investors who hold other currencies such as the euro.
Chinese industrial and precious metals shares rose. Hong Kong-listed Jiangxi Copper Co. jumped 3.8 percent. Aluminum Corp. of China soared 4.1 percent.
Hyundai Heavy Industries, South Korea's leading shipbuilder, jumped 3.1 percent and Doosan Heavy Industries & Construction rose 1.9 percent.
Sentiment was also boosted by speculation that Germany would be willing to agree to an increase in Europe's bailout fund to ?700 billion ($930 billion). Germany has to date resisted calls to increase the lending capacity of the fund beyond the planned ?500 billion ? despite uncertainty over the ability of Rome and Madrid to repay their debts.
Benchmark oil for May delivery was up 12 cents to $107.15 in electronic trading on the New York Mercantile Exchange. The contract up 16 cents to settle at $107.03 per barrel in New York on Monday.
In currency trading, the euro rose to $1.3347 from $1.3343 late Monday in New York. The dollar slipped to 82.79 yen from 82.84 yen.

At McDonald's, 24 percent of its frequent customers report being either unhappy or very unhappy with their health.

While most Americans are happy with their family, friends and home life, more than one in five Americans are unhappy with their health.
According to a new survey conducted by consumer-focused information site BIGinsight, those who frequent certain fast-food restaurants are even less happy with their health than the average American. 24/7 Wall St. examined the results of the survey to identify the fast-food chains with the most-unhealthy customers.

Customers of four of the eight largest fast-food restaurants report a higher rate of being either totally unhappy or somewhat unhappy with their health than the general population. Not surprisingly, the fast-food diners who are most unhappy with their health also report other relatively unhealthy habits. Customers of three of the four restaurants also report among the lowest rates of exercising regularly among all major fast-food chains. McDonald?s, whose customers are the most unhappy with their health, has the lowest rate of customers who exercise regularly.
The four restaurants with the most-unhealthy customers also have the lowest rates of customers who watch their caloric intake. Three have among the lowest rates of customers watching their fat intakes. This behavior does not mix well with a diet that includes a number of fast-food meals.
However, not all fast-food restaurants have unhealthy customers. Chains where customers exhibit healthier behaviors, such as Arby?s, Chick-fil-A and Subway, have customers with above-average levels of happiness with their health. As Consumer Insights Director Pamela Goodfellow told 24/7 Wall St., ?The difference between these consumer groups lies in what they do beyond the drive-thru: exercising, counting their calories, focusing on fat intake and watching their health in general.?
These are the fast-food chains with the most unhealthy customers.
1. McDonald?s
Except for Chick-fil-A, none of the fast-food restaurants skew so heavily female, with women representing 56?percent of the company?s regular customers. An alarming 24 percent of the chain?s frequent customers report being either unhappy or very unhappy with their health. Almost 30 percent of the restaurant?s regular customers report avoiding all the healthy habits measured by BIGinsight: watching calories, fat, salt or carbohydrate intake, or exercising regularly. This compares to 20 percent nationwide who report avoiding these healthy habits. McDonald?s has attempted to improve its image in recent years. It has begun automatically including apple slices and reducing the size of the fries in every happy meal.
2. Taco Bell
Taco Bell?s customers have the lowest rank in a number of health-related metrics measured by BIGinsight. Less than 31 percent of the customers report that they watch their fat intake, and only 19 percent watch their carbohydrate intake. The restaurant is second-worst when it comes to customers watching their calorie intake, as well as their salt/sodium intake. There are ways to eat healthily at Taco Bell. By ordering items ?Fresco Style,? the sauce and cheese are exchanged for a ?fiesta salsa.? Many items frequently considered healthy, such as salads, are among the restaurant?s highest-calorie items.

3. Wendy?s
According to the report, 22.1 percent of Wendy?s?regular customers are either unhappy or very unhappy with their health. About 40 percent exercise regularly. The average household income of a frequent Wendy?s customer is $60,468, the second-highest of the eight major fast-food chains, behind only Chick-fil-A. Just 22.6 percent of the restaurant?s frequent customers are single adults, second-lowest behind Arby?s in this category. Wendy?s offers healthy choices, including apple slices and salads. Even the salads, however, can be unhealthy, with high sodium and saturated fat.
4. Burger King
The demographics of the diners who frequent Burger King are unique in two major ways. A majority of restaurant?s customers ? 58.6 percent ? are men, the highest of all fast food chains. Also, the restaurant is frequented by customers with the lowest average annual household income among fast-food chains, at $52,958. Burger King?s customers watch their caloric intake the least among major fast-food chains, at just 26.6 percent. Only 32.4 percent of the restaurant?s customers watch their fat intake, the second lowest percentage. The company has made efforts to provide healthier menu items over the past year, as an attempt to expand its customer base. It continues to offer fattening classics, however, such as the 1,140-calorie Triple Whopper.
